An original Civil War document at the top it says, "Abstract Of Provisions issued from the 1st day of August, 1863, to the 31st day of August, 1863, to Sick in Hospital at Camp in the Field, under charge of I. J. Clark Surgeon 12th Regt P.R.V.C. by Lieut Samuel Evans 5th, Regiment of P. R.V.C. , A.C.S." Right below it gives account information for various supplies such as pork, cofee, sugar, fresh beef, potatoes, etc. At the bottom it is signed by Issac Clark of the 12th Pennsylvania Regiment and also a Colonel of the 5th Pennsylvania Regiment.
The document is in very good condition, with some small tear marks on some of the fold areas nothing larger than a inch.
