An original Civil War document at the top it says, "Provision Return for Captain H.W. Peck Company 'D' Heavy Artillery Commencing Dec 4, 1863 and Ending Dec 7 1863." Right below it gives account information for rations at Camp Washburn. It is signed at the bottom and on the back it says, "Co. 'D' 1st Wis. H.A."
The document has some wear there are some small holes or chip marks in a few areas, some light stain marks, these flaws do not affect the reading of the document.
