An original Civil War document at the top it says, "Summary Statement of Funds received, disbursed, and transferred at Camp in the Field, in the Month of September, 1863, by Lieut Samuel Evans 5th Regiment of P.R.V.C and, A.C.S." Right below it gives account information. It is signed at the bottom by Lieutneant Samuel Evans.
The document is in very good condition.
